CACT is currently looking for an individual to join their team as an Inclusion and Transition Officer. In this role, you will collaborate with another officer to organize, enhance, and implement a program aimed at assisting young adults between 16 and 25 years old with specific disabilities in accessing various services, such as Young Greenwich and other universal services. The primary focus will be on supporting these individuals and their families, along with engaging with support services and community resources to facilitate their transition from youth services to adult community environments.

  • Coordinate, develop, and deliver programs for young adults with targeted disabilities
  • Prepare and support individuals in transitioning to adult community settings
