Job Title: Community Clinical Support Worker - Dementia Intensive Support Team - West Suffolk

An amazing opportunity is available to join the West Suffolk Dementia Intensive Support Team. You will become part of a dedicated team striving to achieve positive outcomes for individuals and their caregivers.

The team operates in the community, providing support in individuals' homes to prevent care breakdowns and promote independence. The role involves assisting individuals in maintaining their independence by establishing trust, facilitating appropriate care, and managing work autonomously.

  • Undertake work as guided by Practitioners with a specific and clear role from the outset.
  • Build relationships and engage with individuals and their families.
  • Provide intensive short-term goal-orientated input to the individuals.
  • Explore and better understand the needs and risks of individuals.
  • Support individuals in accessing local community resources.
  • Demonstrate skills to other carers/family members.
  • Be flexible and adaptable in the support provided.
  • Highlight any concerns or risks to the responsible Practitioner.
  • Document visits and input into the computer system promptly.
  • Effectively manage own workload.

Job Responsibilities:

Similar to the main duties of the job, the clinical support worker's primary role is to engage and build relationships with service users and their families to introduce care and support to enable them to stay in their own homes. This involves assessing needs, identifying risks, and working autonomously.

Person Specification

Experience

  • Previous experience in the care sector.

Qualifications

  • Care Certificate - must be obtained within the first 12 weeks of employment.
  • NVQ L2/3 in H&SC or Diploma L2/3 in H&SC or apprenticeship L2/3 in H&SC (desirable).

Skills

  • Strong interpersonal and communication skills.

Other

  • Ability to travel independently.

Knowledge

  • Awareness of professional boundaries in relation to client care.
