Job summary

The post holder is responsible for the delivery of practicenursing care to its registered patients. Supported by senior nurses within thepractice, they will deliver care within the boundaries of their role, focusingupon supporting patients to be healthy, monitoring long-term conditions, andengaging in health prevention and screening activities including vaccinations.They will work collaboratively with the general practice team to meet the needsof patients, supporting the delivery of policy and procedures, and providingnurse leadership as required.

Main duties of the job

Assess, plan, develop, implement and evaluate individual careplans to promote health and well-being, and prevent adverse effects on healthand well-being.

Identify and manage as appropriate, treatment plans forpatients at risk of developing a long-term conditionIdentify and manage as appropriate, treatment plans forpatients with a current acute health concern.

Prioritise health problems and intervene appropriately toassist the patient in complex, urgent or emergency situations, includinginitiation of effective emergency care and referral on when appropriate.

Support patients to adopt health promotion strategies thatencourage patients to live healthily, and apply principles of self-care,including taking opportunities to promote health and wellbeing wherever theyarise.

Provide information and advice on prescribed orover-the-counter medication on medication regimens, side effects andinteractions.

Support and advise women requesting information relating tofamily planning needs and those presenting for cervical cytology consultations.

Recognise, assess and refer patients presenting with mentalhealth.

Implement and participate in vaccination and immunisationprogrammes for both adults and children- including vaccinations for thosetravelling abroad.
