Job Title: Specialist Nurse in Integrated Frailty Service

An exciting opportunity is available for a dedicated and qualified registered nurse to join the Elderly virtual ward team at Bradford Teaching Hospitals. As a Specialist Nurse in the Integrated Frailty Service, you will play a crucial role in delivering comprehensive care to frail older patients in a community setting.

  • Manage caseloads and allocate visits to staff efficiently
  • Work closely with a multidisciplinary team including Consultants, Geriatricians, and other healthcare professionals
  • Participate in team meetings and contribute your expertise

Person Specification:

Experience

Essential

  • Extensive experience with frail older patients in a community/ward setting
  • Previous experience in teaching, assessing, or supervising in a clinical environment

Desirable

  • Familiarity with Systmone/IT systems
  • Demonstrated team management skills

Skills

Essential

  • Effective communication and interpersonal skills
  • Ability to handle sensitive information with discretion
  • Proficient in planning, implementing, and evaluating care programs
  • Proven teamwork abilities

Desirable

  • Capability to manage challenging situations
  • Basic computer skills including Microsoft Word and EPR

Knowledge

Essential

  • Stay updated on current clinical and professional practices
  • Understanding of evidence-based practice and research
  • Knowledge of clinical governance and risk management

Desirable

  • Awareness of resource management principles

Qualifications

Essential

  • Registered Nurse with 1st Level qualification
  • BSc/1st degree in a health-related field or equivalent
  • Mentorship/teaching qualification
  • Continuous professional development evidence

Other Requirements

Essential

  • Hold a UK Driving license and have access to a car for work purposes
  • Demonstrate reflective practice in your own professional development
