Discharge Facilitator Specialist

We are pleased to offer a permanent Band 6 Discharge Facilitator post working within Abdominal Medicine and Surgery (AMS). AMS is a large unit within St James Hospital comprising a dedicated surgical assessment unit and 12 in-patient wards.

We are a regional centre for urology, colorectal, gastroenterology, upper GI, HPB and renal services, as well as being responsible for the Leeds Liver Unit, one of a handful across the country.


As a regional specialist centre our patients can often be extremely complex with a multitude of post-operative care and rehab demands.

And due to our services being in high demand it is the responsibility of the discharge facilitators to ensure that once treatment has been completed, patients can be safely and efficiently discharged or repatriated back to the referring hospitals.

Organisational skills and an ability to effectively manage a caseload independently are a must.

As are excellent communication skills and an ability to interact with not only all members of the multi-disciplinary team, but also patients and family members at often difficult and distressing times in their lives.
