The Unplanned Care Team is dedicated to providing support for unplanned nursing situations and organizing catheter clinics within the community. Main Duties of the Job

- Conducting unplanned care visits to support the Community Nursing teams across West Place
- Contributing to the delivery of top-tier healthcare services to patients in various settings by assessing, planning, implementing, and evaluating care packages

  • The role will involve roughly 20% of triaging incoming community nursing referrals, assessing patients' needs, prioritizing care, and handling referrals accordingly.

This is a recently formed team operating within Triage in West Place, comprising Triage Nurses and allocators led by a Clinical Operations Manager. We strive to offer exceptional face-to-face urgent care to patients, ensuring that safe, pertinent, and prompt care is extended to the local homebound population of West Norfolk. We collaborate closely with our colleagues from Community Nursing, GPs, and Social Services.
