The role will involve working as part of a multi-disciplinary frailty team helping to rationalise and optimise medicines and developing prescribing systems to ensure safe, effective and efficient prescribing within the primary care network.
You will have strong experience in a primary care setting and be confident in liaising with patients and other health professionals
You will have the ability to interpret complex clinical information and present in suitable formats to a variety of people from professional and non-professional backgrounds.
Experience of implementing and promoting cost effective strategies as well as previous safety audits and similar work will be an advantage, as will knowledge of the PCN Direct Enhanced Services and supporting pharmacists around structured medication reviews for frail older people.
Established on 1st October 2019, the Trust is creating a centre of excellence in providing rural and remote healthcare and provides a wide range of community and acute services throughout north Cumbria and beyond.

Disclosure and Barring Service:

Standard Check:
£18 + Administration cost of £3.85 = £21.85. Deducted at £6.85 from 1st month salary and £5 deducted from salary over following 3 months.
Enhanced Check:
£38 + administration cost of £3.90 = £41.90. Deducted at £11.90 from 1st month salary and £10 deducted from salary over following 3 months.
