Administration - Care Coordinator

gtd healthcare_ is one of the largest NHS commissioned Primary and Urgent care providers in North West England, we have excellent opportunities for Administrative Coordinators roles to support our urgent care services at Royal Preston Hospital.


We have full time & part time night shift roles available with enhanced pay for out of hours working covering Monday to Friday and Saturday & Sunday.

Night Shifts usually are 10pm to 8am working hours.

As a Care Coordinator for _gtd healthcare_, you will be the first point of contact for health care professionals, patients and patients' relatives accessing NHS urgent and non-urgent care services across areas of North West England


Under the direction of the senior care coordinator, clinical shift lead and within your team, you will register patients onto the relevant clinical system and direct them to the most appropriate healthcare service/pathway contributing to a 24/7 Care Coordination Service.


Key duties include:

  • To deliver a high quality health care coordination service to patients and health care professionals
  • Accurately record on a range of clinical systems all relevant data for each contact and direct appropriately
  • Undertake the booking of appointments across a number of health care services
  • Liaise with the senior care coordinator and clinical shift lead regarding any operational issues affecting the flow of patients and ensure excessive waiting times do not occur
  • Coordinate the arrangement of any followup care as directed by the senior care coordinator or clinical team
  • Liaise with the multidisciplinary team to ensure the smooth running of the service
  • Liaise with NHS 111 service regarding patients who are out of area, duplicate calls or incomplete information
