- gtd healthcare_ is one of the largest NHS commissioned Primary and Urgent care providers in North West England, we have an excellent opportunity for a Senior Administrative Care Co-ordinator position to support our urgent care services at Royal Preston Hospital and Chorley Hospital.


As a Senior Care Co-ordinator for _gtd healthcare_ you will support the operations manager and leadership team on a daily basis to ensure safe service delivery.

You will supervise the team of care co-ordinators and drivers, lead on inductions, training and ongoing development. The role also involves participating with the day to day operational challenges.


Operating within clearly defined limits of policies, procedures and guidelines of the service, this post carries responsibility for ensuring patient care is efficiently coordinated 24/7.


The post holder will work with Clinical Shift Leads and a range of health care professionals to coordinate the travel of patients through urgent and non-urgent care services across a number of ICB areas.


Key Duties and Responsibilities

  • Supervise a team of Care Coordinators, ensuring excellent patient care is maintained
  • Participate in induction, training and ongoing professional development of the Care Coordination Team
  • Ensure that the local agreed targets for the services are met across areas of responsibility
  • To deliver a high quality health care coordination service to patients and health care professionals
  • Accurately record on a range of clinical systems all relevant data for each contact and direct appropriately
  • Undertake the booking of appointments across a number of health care services
  • Liaise with the Operational & Clinical shift lead regarding any operational issues affecting the flow of patients and ensure excessive waiting times do not occur
  • Liaise with the multidisciplinary team to ensure the smooth running of the service
  • Liaise with NHS 111 service regarding patients who are out of area, duplicate calls or incomplete information
