Job summary

This is an exciting opportunity for a clinical fellow to join the Obstetrics and Gynaecology department at University Hospitals Birmingham.

The department is seeking to appoint a Clinical Fellow for Acute Gynaecology and Ultraound scanning, ST5 and above equivalent. This post can commence as soon as possible and will be for a period of one year, potentially renewable.

Applicants should have experience at working at a ST5 or above level for a minimum of 6 months within the UK. Please refer to the RCOG Curriculum for training matrix.

Candidates will require current and valid GMC registration.

Main duties of the job

At the ST-5 level, the post-holder will be expected to have completed competences in line with RCOG recommended competencies for ST4 and have completed MRCOG Part 1. AT ST6-7 level, the post-holder will be expected to have completed competences in line with the RCOG recommended competencies ST5 and MRCOG Part 3.

At ST5 level, the post-holder will be supported to work towards/complete the optional/ intermediate Ultrasound Modules on Gynaecology and Early Pregnancy. The job will include at least 2 regular supervised sessions per week in ultrasound scanning, regular sessions in Gynaecology assessment unit, GOPD 2ww clinics, theatres (as and when needed), session on ward 2 GHH for elective/ emergency care. The fellow would be expected to lend support to any appropriate MDT meetings and other meetings in Gynaecology.

For ST6-7 level, the post holder will have an opportunity to undertake training for the Advanced Specialty Training Modules (ATSM)/ Special Interest Training Modules (SITM) and support achievement of CESR.

The successful candidate will take part in an emergency on-call rota and routine elective work in both Obstetrics and Gynaecology. This will include Caesarean Section lists, gynaecology theatre lists, administrative and educational tasks and general and specialist outpatient clinics. On call frequency is 1:8.

Job responsibilities

The post holder will be expected to:

*Work with a named consultant, who will act as a mentor and will be available to perform appraisals, provide support and advice.

*Attend weekly meeting to discuss CTG's and interesting cases for learning.

*Attend monthly audit meetings and trainees are encouraged to undertake audit projects.

*Attend the monthly mortality, morbidity, audit and governance meetings.

*Attend Postgraduate Education meetings.

*Be actively involved on quality improvement, research and teaching projects to enhance surgical portfolio.

*Be able to liaise with Consultants, other junior doctors, Nurses, Midwives, patient, relatives, administrative staff and other senior medical staff.

*Be able to eventually supervise/ support more junior members of the team.

Duties will also involve but will not be limited to working on the gynaecology assessment unit, labour ward, reviewing gynaecology and obstetrics emergencies and caring for elective gynaecology patients as well as attending gynaecology clinics and participating in elective Caesarean section lists as needed. There will be opportunities to develop special interests in one of the following areas:

*Gynaecology Ultrasound

*Early Pregnancy Ultrasound

*Acute Gynaecology (GAU and clinics)

*Gynaecology theatres (when needed)

GYNAECOLOGY

Gynaecology has 1 ward; ward 2 on the Good Hope Hospital site which manages inpatient elective and emergency activity, alongside a co-located Gynaecology Assessment Unit (GAU), with early pregnancy assessment unit.

On the Birmingham Heartlands site there is also a GAU that is situated in the Princess of Wales Women's Unit and provides clinical care for early pregnancy, recurrent miscarriage and non-pregnant pathways.

At Solihull, the outpatient procedure suite, known as the 'Care 4' Suite, has facilities for colposcopy, hysteroscopy, fertility investigations and treatment, Urogynae clinics, Urogynae diagnostics, vulval clinics, menopause and general gynaecology outpatients.

OBSTETRICS

There are two Obstetrics units serving approximately 9,000 births per annum at Birmingham Heartlands Hospital and Good Hope Hospital. At Solihull Hospital, there is a standalone Birthing Unit which provides midwifery led care for low-risk women.

Birmingham Heartlands Hospital

The Obstetric service at Birmingham Heartlands Hospital is located within the Princess of Wales Women's Unit. There are 46 obstetric inpatient beds situated on the first floor of the unit. The ground floor comprises the Antenatal Clinic, Ultrasound Department, Day Assessment Unit, Delivery Suite and Neonatal Intensive Care Unit (Level 3).

The Delivery Suite has 13 delivery rooms, (including two emergency theatres and two HDU rooms). There is a 4 bedded Pregnancy Assessment and Emergency Room with its own patient waiting area, 6 beds for induction of labour on the labour ward.

There is a midwifery led unit (Willow Suite) comprising 3 rooms with en-suite bathroom facilities. One room has a birthing pool and is predicted to deliver approximately 1,200 women per year.

There is a theatre on the first floor dedicated to elective caesarean section theatre running 5 days per week.

Solihull Hospital

There is a dedicated antenatal clinic, gynaecology clinic and ultrasound department.

Good Hope Hospital

The Maternity Unit includes 1 maternity ward with a total of 24; as well as a Maternity Assessment Centre (MAC) and a Midwifery Led Birthing Unit, which was opened in 2021. There is also a delivery suite, with 10 rooms, and a birthing pool inclusive of 2 theatres and a high dependency room, a dedicated antenatal clinic area, with a comprehensive ultrasound facility integral within the clinic area and a day care antenatal unit. There is also a neonatal special care (level 1) located within the maternity building.

Clinical Duties:

*Routine clinical care of the obstetric and gynaecological inpatients under the supervision of the Consultants. This includes ward rounds, elective and emergency admissions and preparation of patients for theatre.

*Management of patients in labour independently or with some supervision from a consultant or more senior registrar colleague

*Gynaecology Assessment Unit and EPAU

*Attendances in other departments, when requested, for purposes of emergency consultation.
