We are looking for Senior Registered Adult Nurses to work within a community integrated team as part of a multidisciplinary team.
The service provides 24/7 nursing and therapeutic input to patients within their own homes with the aim of returning them to maximum independence, by actively promoting self-management on all activities of daily living.
You will provide a case management approach to care for an identified number of patients working closely with GPs, the ICTs and other Statutory and Voluntary agencies.
This will be achieved by providing advice, information and education to patients, family and carers with the aim of enabling patients to enjoy maximum independence, health and wellbeing.

You will be responsible for the expert assessment, planning, implementation and evaluation of programmes of evidence based nursing care to meet complex health care needs to individuals and their carers within the defined caseload across the community setting.
