We are looking for a dynamic and enthusiastic Physical Health Care Assistant to support the physical health needs of patients with Serious Mental Illness (SMI) in the community.


People with Severe Mental Illness (SMI) suffer from disproportionate physical health conditions compared to people from a similar age and background without SMI.

This post will complete the PH monitoring, checks and interventions for this patient group across Primary and Secondary Care. The checks will be carried out in Team bases and patient homes.

  • Previous experience working in a care or rehabilitation setting
  • Holistic approach to care.
  • Ability to work alone with patients.
  • Ability to work in a variety of settings
  • Good level of secondary education with demonstrable literacy and numeracy skills.
