A exciting opportunity has arisen for an experienced Diabetes Specialist Nurse to join the Coventry community Diabetes Specialist Nursing team.

The Diabetes Nursing Team provides a service for both in-patients and out-patients across three sites University Hospital Coventry and Warwickshire, Rugby St Cross Hospital and Newfield House in Coventry.

The successful candidate will share our commitment to patient centred care. They will be highly experienced, with advanced skills in managing complex diabetes patients in a variety of settings.

They will be able to make autonomous, evidence based decisions and lead by example to educate and support colleagues in primary and secondary care.

The post holder will work to proactively manage, coordinate and support an identified caseload of patients with diabetes. This will include patient and carer education, annual health checks and oversight of insulin management.

Main duties of the job You will be responsible for the direct management of a specialist caseload of patients in both a clinic setting and within the patients home.

To provide assessment and support to patients with complex diabetes needs and advise on appropriate diabetes management.

Apply expert knowledge, advanced skills/practice and autonomous decision making skills.

The role will involve empowering patients with the skills and knowledge necessary to attain independence in self-management.

The role will include operational and specialist responsibilities, providing support, training and advice to community staff, patients, carers and the wider multi-disciplinary team.   • Work proactively manage, coordinate and support an identified caseloads of patients with Including patient and carer education, annual health checks and oversight of insulin management.

  • Participating in providing training of identified staff groups.
  • Utilise and ensure others utilise IT systems to secure accurate and timely patient, workforce and resource data.
  • Support patients, carers and staff during difficult situations arising in the clinical setting.
  • Participating in clinic prep and nurse led clinics.
  • Inputting data and being involved in audits.
  • Promote and monitor adherence to Health and Safety and Trust policy designed to protect healthcare staff and service users from known hazards.
