High Intensity User Lead

The High Intensity User (HIU) Lead is responsible for setting up and establishing, by leading and coordinating, a robust collaborative approach to high intensity users identified within the Somerset system, this will include close working with the high intensity user groups at Musgrove Park Hospital and Yeovil District Hospital Emergency Departments (ED) and complex care teams within the PCNs.
The HIU Lead will work with colleagues across the Somerset system and together identify and reduce HIU activity to ED, non-elective admissions, ambulance, 999 and 111 and primary care.
The Trust provides outpatient and inpatient consultant services for a range of specialisms including general medicine, cardiology, general surgery, orthopaedic surgery, trauma and paediatrics.
- The post will involve communicating with a wide range of professionals and VCSE colleagues and partners across our health and social care system.
- Will hold a caseload of clients, which will require building trust with people who may have a mistrust of healthcare services, have medical and social lives that are highly complex with multiple services involved.
- Working with individual client's families and carers, to develop understanding of their individual situation and support a plan to positively change healthcare use behaviour.
- Clear communication will be expected via face to face and virtual meetings, telephone, letters and electronic communication.
- Providing reports and presentations on service development and outcomes to the ICB and Trust around activity and impact of interventions, which may be sensitive in nature and require engaging communication skills.
- Responsible for chairing the HIU groups (HIUG) at Yeovil District Hospital and Musgrove Park Hospital.
- Routinely working with clinical and nonclinical experts from across the Somerset system to manage conflicting priorities and interdependencies.
- Working with colleagues across the system to raise awareness of the service, identifying individuals who would benefit from the service and ensuring appropriate personalised care intervention.
- Represent Somerset HIU service and maintain effective links with the Southwest HIU collaborative to ensure strong networking links and seizing opportunities for shared learning.
- Ensure that developed Personal Support Plans are shared with other professionals in line with Information Governance Guidance.
