To support a quality service across the Trust, for patients following hip fracture by promoting an integrated pathway that is understood by all involved and meets the complex needs of these patients.


  • To be responsible for continual inputting and updating the requirements of the National Hip Fracture Database.
  • Reporting to managers any identified gaps in the agreed pathway and database which will ensure patients receive quality care with maximum compliance across the pathway.
  • To work as part of the multidisciplinary team, acting as liaison point between Orthopaedic Consultants, theatre staff, ward staff, A&E, Orthogeriatricians, therapists and trauma coordinator.
As per job overview and including;

  • To be responsible for managing own time management and prioritising workday.
  • Ensure the timely and accurate inputting of data on the patient information management.
  • Audit the progress of the hip fracture patients and complete the NFHD tool with attention to detail and accuracy and produce reports as requested and present to Fragility Hip Fracture Monitoring group.
  • To liaise with the data quality department to report any anomalies' found in any patient data on the trusts clinical systems, highlighting errors or concerns.
  • Regularly attend and contribute to the hip fracture monitoring group and the MDT mortality review group.
  • Regularly monitor #NOF patients across the hospital and ensure that the MDT are aware and that the outlier ward fully understand and utilise the integrated pathway.
  • Deal directly with queries from other health care professionals and visitors and refer to appropriate MDT fracture NOF pathway team member where necessary.
  • Identify systems for sharing good practice within the ward/department and Trust wide.
