This role includes clinical triage of patients referred into community services The identification, assessment, planning, implementing and evaluating care as a specialist practitioner.
Including the completion of specialist discharge planning and specialist nursing or therapy assessments, trusted assessor assessments, equipment ordering in accordance with local and National guidelines Provide advice and clinical expertise to other colleagues including those from acute trusts, CPFT inpatient units, other CPFT services and nursing and residential homes to expedite discharge from patient pathways and where possible prevent admissions Make recommendations for health care packages for patients and assess the need for referral to other agencies and disciplines as appropriate
