About the Role
We are seeking a dedicated and passionate Health and Wellbeing Practitioner to join our team at The Forward Trust Careers. As a national organisation, we are committed to providing opportunities to all people, including those with lived experience, and we encourage applications from diverse backgrounds.
Key Responsibilities
- Deliver Personal Wellbeing Services to adult male prison leavers and those on community orders.
- Work remotely in mandated hubs and delivery locations as part of a team of practitioners and subcontractors.
- Develop and deliver a range of programmes tailored to meet individual needs, focusing on Family and Significant Others, Lifestyle and Associates, Emotional Wellbeing, and Social Inclusion.
- Collaborate with colleagues to provide a comprehensive and holistic package of support to reduce re-offending.
Requirements
- Excellent people skills and a commitment to providing person-centred support.
- Ability to work independently and as part of a team.
- Desirable: full UK driving licence for travel across the region.
