About the Role

We are seeking a skilled Health and Wellbeing Practitioner to join our team at The Forward Trust. As a Health and Wellbeing Practitioner, you will play a vital role in supporting adult male prison leavers and those on community orders in Cambridgeshire.

Key Responsibilities
  • Deliver Personal Wellbeing Services to support individuals in achieving their goals and improving their overall wellbeing.
  • Manage a diverse caseload, conducting comprehensive assessments and developing collaborative action plans with service users.
  • Deliver structured one-to-one and group work sessions, adhering to relevant manuals and guidelines.
  • Maintain accurate records and review ongoing support with other agencies, including probation services.
  • Meet key performance indicators (KPIs) and deadlines.
Requirements
  • Knowledge of the issues facing service users in the criminal justice sector.
  • Experience working in Probation, prison, or personal wellbeing services, including families, lifestyles, emotional wellbeing, and social inclusion.
  • Demonstrable experience managing high caseloads, conducting risk assessments, and creating SMART action plans.
  • Ability to deliver one-to-one and group work interventions using motivational techniques.
  • Quick thinking, high initiative, and excellent communication skills.
  • A relevant qualification in criminal justice or social care, or equivalent experience.
