Community Nursing are the beating heart of our health and care services - we look forward to you working with us . Main duties of the job The post holder will work as a complex case manager, without direct supervision. You will be involved in delivering integration, working with GP's, Social services and the third sector organisations. You will be providing case management, facilitating early discharge from hospital and preventing unnecessary/unplanned hospital admissions where appropriate as well as supporting healthcare closer to home. The service currently operates from hrs, Monday to Friday.The post holder will be expected to support their team, department and organisation to achieve the Trust's Values in their day to day work.The post holder will be a Registered Nurse, with at least 2 years community experience of managing a complex caseload of patients.You will be responsible for a caseload of patients with complex health and social care needs. You will work autonomously to case manage these clients and use advanced nursing skills, such as assessment and prescribing to manage their conditions in the community. They will work as part of the integrated care team, working within and across professional and organisational boundaries. Liaising with GPs, other professionals and statutory and voluntary agencies he/she will deliver individualised care to patients. We recognise that taking the vaccine provides the best defence against COVID 19 for our patients, our staff and their families. Probationary Period This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff). Starting with NELFT NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex. High Cost Area Supplement - London This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (minimum payment of 4,313 and a maximum payment of 5,436 per annum pro rata).
