A rare and exciting opportunity has arisen for an extraordinary person to help us transform the way we support people living with cancer in Lincolnshire.


The number of people in the UK living with cancer is expected to rise to 4 million by 2030, with 45400 living in Lincolnshire alone.


Lincolnshire people affected by cancer have told us that they have unmet needs, particularly at the end of treatment, whilst others are struggling with consequences of treatment that could be either avoided or managed.

The top three things that people living with cancer in the county have told us which have prevented them from living well after a cancer diagnosis are the side effects, and health worries associated with cancer including fatigue and mental health concerns.


We aim to change this by transforming the way we support people living with cancer in Lincolnshire and have recently secured funding from Macmillan Cancer Support for this two year fixed term post to enhance the award winning Living with Cancer programme, and ensure that personalised care and support is available to all people affected by cancer in the county.

This role will lead the transformation of information, advice and support in communities across Lincolnshire and will support the actions to be taken as a Lincolnshire Integrated Care System to deliver the objectives outlined NHS Long Term Plan 2019 and the Lincolnshire NHS Forward View

The Macmillan Clinical Facilitator will:

  • Scope the needs for and gaps in information, advice, and support on LWC across communities in Lincolnshire
  • Offer cancer specialist information to health professionals within communities.
  • Improve access to in person and digital support opportunities
  • Develop a menu of different delivery options to meet expressed needs.
  • Develop local volunteers/champions within potential partner organisations and communities
  • Scope and develop the introduction of Macmillan EHNAs in the community and improve information flow between acute and community settings.
  • Work with the wider cancer team and acute and primary care services to make sure the needs identified by our population are more effectively met
  • Provide specialist clinical advice as required to health professionals working across the system.


NHS Lincolnshire Integrated Care Board (ICB), is a statutory organisation bringing the NHS together locally to improve population health and establish shared strategic priorities within the NHS.

The ICB is part of a wider Lincolnshire Integrated Care System which has four key objectives:

  • Improve outcomes in population health and healthcare.
  • Tackle inequalities in outcomes, experience, and access.
  • Enhance productivity and value for money.
  • Help the NHS support broader social and economic development.


By working together with the ICB, the wider system will deliver its agreed vision of 'Better Lives for the people of Lincolnshire'.

Lincolnshire ICB will use its resources and powers to achieve demonstrable progress on these aims, collaborating to tackle complex challenges including:

  • Improving the health of children and young people.
  • Supporting people to stay well and independent.
  • Acting sooner to help those with preventable conditions.
  • Supporting those with longterm conditions or mental health issues.
  • Caring for those with multiple needs as populations age.
  • Getting the best from collective resources so people get care as quickly as possible.


The Macmillan Clinical Facilitator will be responsible for scoping the needs for advice, information and support and the development of a model of delivering that support for cancer patients, in the community, with their holistic needs before, during, and post treatment, for those living with and beyond cancer and those on a palliative pathway.

Information support and advice development

  • Scope the needs for and gaps in information, advice, and support on LWC across communities in Lincolnshire
  • Offer cancer specialist information to health professionals within communities.
  • Improve access to in person and digital support opportunities
  • Develop a menu of different delivery options to meet expressed needs.
  • Develop local volunteers/champions within potential partner organisations and communities
  • Scope the introduction of Macmillan EHNAs in the community and improve information flow between acute and community settings.
  • Initiate test projects to explore opportunities for the above.
Leadership and Communication

  • Work with the wider cancer team and acute and primary care services to make sure the needs identified by our population are more effectively met
  • Work closely with the Macmillan LWC Community Development Programme Manager to ensure that the LWC Community Development Programme is delivering on key performance objectives
  • Have regular contact with internal and external stakeholders, such as primary care, social care, third sector, and GPs.
