An Exciting opportunity has arisen for a Health Visitor or School Nurse to join the Sheffield Children's NHS Trust 0-19 Service as a specialist SEND Practitioner.


The post holder will lead health visiting and school nursing practice in providing specialist support to health visitors and school nurses in Sheffield.

The post holder will develop pathways for families with children with special educational needs and disabilities (SEND).

You will co-ordinate and provide learning opportunities, expertise, and guidance for Health Visiting and School Nursing teams to optimise and safeguard health outcomes for children with disabilities and you will be required to develop high-level specialist skills in line with local and national SEND Public Health outcomes.


The specialist Health Visitor will work alongside the 0-19 Team Leaders and Specialist Vulnerability Practitioners to identify SEND children and ensure provision of enhanced health intervention based on assessment of identified needs of the child and their family.

22.50 hours per week.


Maintain strong links with partner agencies to ensure that practice developments are in line with identified trends and gaps in service provision particularly for children on the neurodiverse pathway.


Development and delivery of packages of care and pathways relevant to this area of work and support training and implementation of packages of care by practitioners in the 0-19 teams.

Provide and manage a comprehensive service to a defined population.

You will hold your own caseload and contribute to developing practice and raising awareness and safeguarding of those at risk, whilst leading practice and acting as a role model for your colleagues.


Provide supervision and management support for the 0-19 practitioners including skill mix staff in conjunction with the Team Leaders and Specialist Vulnerability Practitioners.


Work closely with Health Visitors and school nurses who have caseload responsibilities for children with SEND, ensuring they have sufficient knowledge, support and understanding of the condition to offer the most effective, evidence-based child centred care.

To have oversight of the numbers and geographical spread of SEND children across the city.


Ensure robust assessments of incoming referrals, clinical decision making, and the delivery of high-quality services for the children, and family from 0-19 years of age.

Work alongside 0-19 Band 7 practitioners to establish effective team peer supervision embedding a culture of team learning related to SEND case work.


Identify roles that can be appropriately delegated to the skill mix team and to use expertise at the right level for effective service delivery.

Develop and embed the role of the SEND ambassador within the 0-19 service.


Work closely with Early Years Services, partner agencies and specialist practitioners to ensure a strong and coordinated service/support for those children and families identified with SEND.

Monitor and evaluate the outcomes of allocated work to the 0-19 teams.

Motivate, inspire, and promote the 0-19 service, creating a consistent focus on the need of children and families.

Monitor and evaluate the performance and the standard of service delivery.

Implement changes to practice through training and supervision (clinical and restorative) of others in the 0-19 teams.


Work with all Band 7 Leaders/Specialists in the 0-19 team to ensure that the 0-19 service specification is delivered to all families.

Act as a clinical expert who is able to assess develop and deliver relevant care packages independently and appropriately.

To access clinical, management and safeguarding supervision in line with local policies and procedures.

To work in line with local safeguarding policies and procedures.


Communicate and work in partnership with clients, colleagues, and other statutory and voluntary agencies where appropriate, receiving, analysing, and presenting complex and sensitive information.
