Job Summary

We are seeking a highly motivated and enthusiastic Public Health Staff Nurse to join our 0-19 Service team in Sheffield. As a key member of our team, you will work closely with children, young people, and families to deliver the Healthy Child Programme and promote health and wellbeing in the community.

Main Responsibilities
  • Deliver the Healthy Child Programme under the supervision of a Specialist Community Public Health Nurse (SCPHN)
  • Conduct routine home visits and monitor and evaluate care
  • Work with children, families, and specialist services to assess needs and plan specific care
  • Support the 0-19 Team to meet safeguarding needs of children and families, working according to safeguarding guidelines
  • Promote the health of looked-after children, including completion of review health assessments
  • Contribute to the planning, implementation, monitoring, and evaluation of therapeutic programmes to empower individuals to make informed choices

Person Specification
  • Post-registration experience of working with children and/or adults
  • Evidence of multi-disciplinary team working
  • Understanding of the Healthy Child Programme
