The Trust's palliative care service is responsible for delivering high quality end of life care to patients and carers based in the acute hospital setting, so if you thrive on a challenge and feel you can make a real difference to the care of our patients, this post may be for you.


The Macmillan End of Life Assistant Practitioner will form an integral part of the multidisciplinary team approach in caring for the physical, social and psychological needs of patients diagnosed with a life limiting illness (both malignant and non-malignant disease) and their carer's.

Ealing Hospital:
a busy district general hospital providing a range of clinical services, as well as 24/7 emergency department and urgent care centre, and specialist care at Meadow House Hospice

Central Middlesex Hospital:

our planned care site, hosting a range of surgical and outpatient services and collocated with an urgent care centre.


We are a university teaching NHS trust, in recognition of the important role we play in training clinicians of the future and bringing the benefits of research to the public.


The overall purpose of this post is to support the delivery of the End of Life Care Strategy within LNWH.

The post holder will be involved in the delivery of nursing care for patients in the last phase of life with the level of knowledge, skill and competence beyond that of a traditional healthcare assistant.

You will support and contribute to education and training of registered nurses, healthcare assistants and other members of the multi-disciplinary team (MDT), with regards to the specific needs of patients and their families/carers at the last phase of life.


Working under indirect supervision, as part of the nursing team, you will have responsibility for delivering nursing care to patients within a delegated caseload.

You will participate in multidisciplinary working and the development, implementation and evaluation of the delivery of health care services.
