This is an exciting period for involvement in Social Prescribing, recognized as a vital element of NHS England's Universal Personalised Care initiative outlined in their Long-Term Plan, as well as London's Health Inequalities Strategy for fostering healthy communities. Both strategies acknowledge the significant role of social, emotional, economic, and cultural factors in health and well-being, emphasizing the importance of community connections in enhancing health outcomes, self-care, and the management of long-term conditions.

Key Responsibilities

We are particularly interested in candidates who reflect the diverse communities we serve. We encourage applications from individuals who are bilingual. You will be an integral part of a Primary Care Network Multi-Disciplinary Healthcare Team, providing individualized support and care planning to clients, employing a holistic, systematic, and strengths-based approach to identify broader issues affecting their health and well-being. Together, you will co-create a well-being plan to address these needs. Strong holistic assessment skills, along with excellent interpersonal and communication abilities, are essential for building trusting relationships that empower individuals in their health journeys.

You will establish robust connections with local Voluntary, Community, and Social Enterprises, as well as community and neighborhood networks, enhancing existing resources to develop a comprehensive array of community groups and assets for individuals to engage with as part of their well-being plans.

Being an attentive listener and having experience working positively with individuals facing complex social and emotional challenges is crucial. While familiarity with the impact of cancer is beneficial, it is not mandatory. A creative mindset and a solution-oriented approach to overcoming barriers to health and well-being are essential.

About Family Action

Family Action is honored to be the provider of the local Social Prescribing Service commissioned within the community, having delivered services since 2014 and recently recognized for excellence in the Social Prescribing Awards. We are thrilled to expand our offerings through a partnership with Macmillan Cancer Support. Successful candidates will become part of the Macmillan professional network.

Job Duties

The role involves collaborating with stakeholders to enhance the visibility of the service within the community, thereby strengthening community resilience and capacity. You will work alongside local agencies to promote awareness of social prescribing, demonstrating how collaborative efforts can alleviate pressure on statutory services, improve health outcomes, and facilitate a holistic, personalized approach to care.

Core Tasks and Responsibilities:

  • Conduct comprehensive assessments and collaboratively design a detailed Care and Support Plan, identifying support needs to maximize engagement in health improvement.
  • As part of the multi-disciplinary team, cultivate relationships with staff in local GP practices, participate in relevant meetings, and provide feedback on social prescribing initiatives.
  • Ensure continuity and a coordinated experience of care for service users, remaining a consistent point of contact throughout their social prescription journey.
  • Support the delivery of personalized care for all individuals diagnosed with cancer.
  • Encourage self-referrals and engage with local communities, particularly those underrepresented in this service, serving as a friendly resource for health, well-being, and preventive strategies.
  • Assist individuals in identifying broader issues impacting their health and well-being, such as financial difficulties, inadequate housing, unemployment, loneliness, and caregiving responsibilities.
  • Collaborate with individuals to co-create a straightforward personalized support plan addressing their health and well-being needs based on their priorities, interests, values, and motivations.
  • Where appropriate, facilitate introductions to community groups, activities, and statutory services, providing follow-up support as needed.
  • Maintain an understanding of and comply with Family Action's procedures for safeguarding the welfare of children and vulnerable adults, seeking guidance for any patient-related concerns.
  • Develop and maintain effective relationships with stakeholders, including health, voluntary, social, and educational resources, attending necessary meetings.
  • Encourage service users connected to community support through social prescribing to volunteer and contribute to others, enhancing their skills and confidence while strengthening community resilience.
  • Solicit feedback from service users and their families regarding the impact of social prescribing on their lives.
  • Engage in ongoing personal and professional development, participating in regular supervision and seeking feedback on service quality.
  • Establish and maintain comprehensive data and evaluation systems, ensuring accurate reporting and compliance with relevant guidelines.
  • Participate in Family Action and other organizational meetings and events to promote and celebrate the work of the service.

Qualifications and Experience

Essential:

  • Professional qualification in Health/Social Care/Mental Health at Degree level, with a commitment to ongoing professional development.
  • Proven experience utilizing various therapeutic approaches, including Motivational Interviewing, CBT, and Solution-Focused Therapy.
  • Strong holistic assessment and consultation skills, with experience providing empowering support to adults.
  • Comprehensive understanding of social and emotional determinants of health.
  • Knowledge of long-term health conditions and strategies for improving health and well-being.
  • Familiarity with welfare benefits systems and housing resources for effective signposting.
  • Experience working in a multi-disciplinary team with strong communication and relationship-building skills.
  • Excellent knowledge of community resources and the ability to organize information for diverse service users.
  • Strong record-keeping and written communication skills, with the ability to produce comprehensive reports.
  • Proficient IT skills for administration and data management.
  • Professional demeanor when engaging with various stakeholders.
  • Understanding of safeguarding policies and procedures for vulnerable groups.
  • Ability to work independently, prioritize tasks, and adapt to new working models.

Desirable:
