About the Role

We are seeking a highly motivated and compassionate individual to join our Specialist Palliative Care team as a Macmillan End of Life Assistant Practitioner. This is an exciting opportunity to make a real difference in the lives of patients and their families.

Key Responsibilities
  • Assist in providing high-quality care to patients entering the end of their lives, including patient and carer assessment, generic staff training, and liaison with treating teams.
  • Support the delivery of the End of Life Care Strategy within LNWH, working under indirect supervision as part of the nursing team.
  • Participate in multidisciplinary working and the development, implementation, and evaluation of healthcare services.
  • Deliver nursing care to patients within a delegated caseload, contributing to education and training of registered nurses, healthcare assistants, and other members of the multidisciplinary team.
Requirements

To be successful in this role, you will need to be a strong patient advocate with good communication and clinical skills. You will also need to be able to work effectively as part of a team and contribute to the delivery of high-quality patient care.
