We deliver acute hospital, primary, community and mental health care services to a population of over 1.15 million and a wider population of 2.2 million when our regional and national services are included.

The shift pattern is

An opportunity has arisen for a Health Care Support Worker B3 37.5 hours.

The role includes working as part of the MDT to support the delivery of a specific care plan to the patients within the NE Older Peoples' Mental Health Team as implemented by registered colleagues.

This will require home visits and lone working.

The role will support the Community Psychiatric Nursing staff within the Multi-Disciplinary Memory Clinic to deliver comprehensive, patient centred assessments, delivered in a compassionate manner.
