Failsafe is a mechanism which ensures that the screening service is accountable for its actions and to ensure that processes are in place so that patients do not come to any harm.

It is also there to ensure that if something does go wrong within the system, processes are in place to identify what is going wrong and to ensure there is a safe outcome.


The Failsafe Officer is responsible for ensuring the raising of risks relating to all patients eligible for antenatal and newborn screening.


To ensure accurate data collection and provision of information to support the smooth running of the maternity service, antenatal and newborn screening programmes, with particular focus on maintaining failsafe systems to enable retrieval of data to demonstrate compliance with UK National Screening Committee requirements.

Essential Responsibilities:


Maintain effective databases for Antenatal and Newborn Screening ensuring that working procedures relating to Data protection are adhered too; provide statistical information to the Antenatal & Newborn Screening Coordinator on a regular and ad hoc basis.


Use own initiative to identify deviations from expected pathways, taking actions to prevent screening incidents where possible and escalating issues as necessary.

Monitor the Newborn screening process to ensure all babies are screened within required timeframe and escalate non-compliance.

Input and maintain audit trails to demonstrate compliance with required Failsafe measures.

Responsible for the designing, improvement, maintenance and monitoring of failsafe administration procedures.

In conjunction with the Antenatal & Newborn Screening Specialist Coordinator:

Continually audit the performance of the programme against national and local standards; design, create and compile specialist reports using Microsoft Excel.


Assist in the preparation and presentation of information for the purpose of reporting, discussing and presenting antenatal and newborn screening data to internal/external parties.

Actively participate in multi-disciplinary team meetings and Programme Board meetings as required.

Additional Responsibilities:


Where necessary to liaise with other departments within the Trust to ascertain and collate necessary information relating to required screening data.
