Job summary

An exciting opportunity to be actively involved in the development of an enhanced Palliative Advice Centre, to support and coordinate care for our patients in the community.

We are seeking to recruit an experienced ANP or CNS with Non-Medical Prescribing to join our well established medical team in delivering exemplary care for people affected by life-limiting illness in both the hospice and community settings.

You will be based in our purpose-built Hospice situated one mile from Macclesfield town centre. There is ample free parking available on site, and subsidised meals are available.

Our service will operate 7 days a week, between the hours of 8am and 8pm.

Main duties of the job

Our new team member will:

Undertake face to face reviews of palliative patients, working across clinical settings, inclusive of Hospice & community work Deliver expert advice to healthcare professionals, patients and relatives both in person and over the phone Coordinate care for people with a life-limiting illness Be confident liaising with other healthcare professionals, making recommendations for changes to medication regimes/plans of care and advocating for patients and their families Be proactive in-service development Deliver evidence based, quality care

We prioritise each patients needs and wishes, aiming to improve overall quality of life with practical assistance, emotional and spiritual support. At a time when family is so important, we also support carers and relatives with social and family services, counselling, complementary therapies and much more.

All of our services are provided free of charge to those who need them. However, only 21% of our funding is from the government and the remainder is donated by the communities we serve: Buxton, Congleton, High Legh, High Peak, Knutsford, Macclesfield, Poynton, Wilmslow and everywhere in between.

Job responsibilities

To assist families affected by dementia in 1:1 and in group settings, to develop skills and competencies in understanding and coping with challenges and difficulties they may face, in both individual and group situations. For example, managing complex conditions, adapting responses when supporting the person with dementia, changes in relationships, distressed behaviour. Direct work with dementia patients and their carers with diverse needs commonly including: nonpharmacological symptom management of fatigue and breathlessness; rehabilitation; falls risk reduction. Holistic specialist assessment, goal setting and intervention for individual patients under the dementia services, with outpatients and in patients own homes. Understand and communicate complex information where emotional, social and spiritual needs are considerable. Use advanced verbal, and non-verbal communication skills to work with patients including those in distress and those who have difficulties understanding or communicating. Consultative role to advise colleagues, patients and carers. Contribute to risk assessments around equipment provision, falls prevention and discharges.

To offer expertise and guidance to professional colleagues, the public and others in supporting family and relationship-centered approaches and best practice in dementia care. To meet the multiple and often complex needs of families affected by Dementia through ongoing assessment of health and wellbeing. To provide a range of psychological and social interventions to promote health and assist with the alleviation, prevention and management of stress, depression, anxiety, and other mental health needs of carers. To maintain clinical records using EMIS and provide verbal and written feedback appropriately and as required, to carers, persons with dementia (as appropriate), referring agencies and professional colleagues. To liaise with GPs, case managers, primary health care practitioners, statutory and voluntary agencies regarding the best possible care for the family affected by dementia. To act as an advocate to facilitate access to services and influence care provision. To identify development options for addressing inequality and to contribute to strategies designed to improve access to services for people with dementia and carers who are socially excluded. To ensure that care is delivered in accordance with relevant local and national standards such as those recommended in NICE guidelines, national dementia and carer strategies/ plans and all relevant East Cheshire Hospice policies Ensure that all legal requirements relating to Mental Capacity Act (2005), Safeguarding and Mental Health Act (2007), and Deprivation of Liberty Safeguards are considered and adhered to. Recognise the limits of own competency and professional boundaries and make appropriate and timely referrals to other services where required. Undertake other duties as may be required and which are commensurate with the post and grade.

Experience

Essential

Minimum 2 years post-registration clinical experience of working with people with dementia and their carers in different settings. Experience of working with groups and individuals in a variety of roles and settings. Experience of assessing patients and providing support with rehabilitation, energy conservation, fatigue management and or falls management. Experience of working with family carers. Assessment and treatment using holistic approaches and/or standardised assessments as appropriate. Highly developed interpersonal communication skills for working with patients in collaboration with a wide range of hospice staff. Demonstrated experience of working as an autonomous practitioner with good organisational skills. Understanding of the principles and practice of quality assurance and service evaluation including audit. Strong computer literacy including Teams, Word, Outlook and Excel.

Desirable

Experience of delivering educational programmes within a healthcare setting. Experience running structured groups. Experience of using EMIS patient system. Palliative care experience.

Qualifications

Essential

Diploma or degree in Occupational Therapy or Physio Therapy. Membership of HCPC. Experience working at Band 6 level. Advanced Communication Skills. Post registration qualification / training in palliative care/or equivalent experience.

Teaching qualification. Palliative/End of Life Care qualification. Membership of BAOT / RCOT. Advanced communication skills training.
