What the Job Involves

Securing and setting up care and support packages for adults with assessed care needs within the local marketplace.
Handling contractual activities by generating appropriate contracts, Service Level Agreements, requisitions, and authorizing payments for services.
Providing advice, guidance, and sourcing services for self-funders as a paid service.
Defining outcomes, setting targets, and monitoring performance within a culture of continuous improvement.
Building strong partnerships internally and externally to deliver services efficiently and effectively.
Using new technologies to adopt modern, agile working practices, improve customer service, and eliminate paper-based processes.
Acting as the primary interface between service providers and the Council for sourcing services as per user needs.
Negotiating with providers to ensure packages of care meet the aspirations and needs of service users while securing value for money.
Collaborating with ASC Operational teams and relevant professionals to monitor cases and broker services in a timely manner.
Liaising with service users and their families to discuss requirements and keep them informed of progress.
Resolving supplier queries and supporting onwards referral and handover to appropriate teams.
Ensuring contracts and Service Level Agreements are signed and implemented correctly.
Maintaining detailed records and systems for monitoring vacancies, allocations, and placements within ASC.
Developing and maintaining good relationships with service providers and enforcing sector-wide service performance improvements.
Managing necessary actions in case of provider suspensions or failure that impact service delivery.
Keeping up-to-date knowledge of services, market demands, and relevant regulations or good practices.
