Support Coordinator
Permanent, Full Time (37.Want to be part of a team that really cares and empowers you to help change a customers life for the better? As our Support coordinator, youll be at the heart of supporting our customers who have complex needs including mental health issues, substance misuse and offending behaviour, to develop their skills to live more independently in the community. Youll make a big difference each day as you help our customers achieve their hopes and aspirations. Our customers live in our main accommodation as well as living in dispersed properties, where we have 10 flats in the community. We aim to help our customers live independently.
Typical day as a Support Coordinator
Creating support plans with your customers and coordinating our team of Support Workers working to the plans.
Supporting our customers who have complex needs including mental health issues, substance misuse and offending behaviour Helping customers with daily living activities such as making meals, shopping, budgeting, maintaining their tenancy and moving onto independent accommodation if they are able.
Carrying out risk assessments, support planning, goal setting and regular reviews.
Cuppas, chats and catch ups. Fancy going home each day knowing that you have helped change our customers lives for the better? Passion to support our customers to live their best life, working collaboratively with an eye for detail.
Experience of creating person-centred support plans and supporting colleagues in working to work to the plans.

Youll work alongside our fabulous team who are super passionate about supporting customers to reach their goals and celebrating their achievements.There are 20of us in the widerteam and we dont just love to support our customers, were there for each other too
Youll work on a rota basis between the hours of8am and 10pm and take your turn workingdays, evenings, weekends and bank holidays.
We need you to be flexible to meet the needs of our customers, but we aim to give you the time you need to spend time doing the things that matter to you
Where were flexible with you, we do need to meet the needs of our customers. Able to use technology for creating and updating support plans, complete online learning and to collaborate with colleagues.
