Job overview

An exciting opportunity has arisen for a Band 7 Discharge Coordinator to work within the Mental Health Acute Crisis Response Service to support the continued development of the service.

The successful candidate will fill in the role of the Mental Health Acute Discharge Coordinator based in Goodmayes Hospital.

The team is looking for a dynamic, proactive, enthusiastic, and highly skilled professional who will form a key part of the discharge process within the service. The post holder will play a lead role in ensuring that all matters related to client discharges or transfer of care from hospital are seamless and of a high standard. This will entail working closely with all professionals, families and carers, including the Police, Voluntary Community Sector Enterprises, Local Authority, Finance Department, Mental Health Community Teams

Applicants will need to have a minimum of three years post-qualification clinical experience in mental health, where a degree of autonomous working is required. They should be able to demonstrate the ability to work on their own initiative, have good time management skills and be able to prioritize workload effectively. Applicants will need to demonstrate well developed leadership and interpersonal skills especially when handling complex cases , and should have the appropriate skills to demonstrate a good knowledge around discharge barriers/ processes. 

Main duties of the job

To work in collaboration with the ward multidisciplinary team, Social Services, and other relevant personnel at both the hospital and community to plan and manage timely and appropriate transfers of care (discharge) for patients from the hospital. To work in partnership with the wards, community teams and external Health agencies in relation to the provision of all aspects of care following discharge. To attend weekly bed management meetings to discuss and support the transfer of care arrangements for patients. To be responsible for identifying potential complex discharges on/ during admission, and in collaboration with the multidisciplinary team set and monitor discharge dates. To identify and escalate delays in discharges to the Acute Crisis Service lead for further escalation to the Assistant Medical Director Working collaborative with the Home Treatment Teams, Community Recovery Team, Access Teams, Inpatient wards, Substance misuse team, Perinatal team , primary services and others services To be able to communicate effectively with clients and their families/carers regarding discharge planning and safety plans.

High-Cost Area Supplement

This post also attracts payment for High-Cost Area Supplement of 15% of the basic salary (minimum payment of £4,313 and a maximum payment of £5,436) (pro rata for part time staff).
