Job summary

We have an exciting opportunity for a Healthcare Support Worker to join our team to deliver the Your Health service, operating Monday to Sunday between 8am and 8pm.

The successful candidates will spend their time carrying out a range of clinical and non clinical tasks including phlebotomy, swabbing and immunising as well as registering patients with their local GP practice, signposting and educating patients, attending clinics and liaising with professionals from a range of multi disciplinary teams, whilst managing their own case load.

You would be working alongside and supporting a Band 6 Registered Healthcare Professional and Team Leader to deliver services in line with Your Health.

If you hold NVQ Level 3 in Healthcare or have a willingness to achieve, have experience of working under distanced supervision, and can demonstrate delivering patient focused care with empathy, sensitivity, tactfulness and diplomacy then we'd love to hear from you.

This role will require travelling between sites and therefore a current full driving licence and access to a car during working hours is essential.

Main duties of the job

Key Areas of Responsibility (refer to JD attached for further detail)

To respond to infectious outbreaks as and when required, inclusiveof swabbing and immunising

To work with all health care professionals, and statutory/nonstatutory agencies to provide a seamless, integrated service to our serviceusers

To deliver high quality care in the community working withpatients towards self-management and independence throughout the continuum ofcare.

To participate in a team of colleagues supporting evidence based care.

To support the Band 6 with stock take of equipment and consumableson the vans.

To undertake daily fridge monitoring and recording for the fridgeon the van.

Tokeep the patient at the centre of his or her care, ensuring patient and carerparticipation in decision making.

The post holder will:

Assist the Team Leader withplanning the delivery of the clinics.

Undertake stock checks dailyensuring the van is adequately equipped for the clinics.

Undertake clinical elements ofthe Your Health services (blood samples / history taking / vaccinating) andrecord data in EMIS and feedback forms.

Where there are barriers tounderstanding, such as hearing impairment, mental capacity impairment and otherdifficulties in comprehension, explanations require adjustments in order togain understanding, consent and concordance.

Seek patient consent and recordthis in the patient record.

Be aware of assistivetechnology and utilise where appropriate.

To participate inmulti-disciplinary/multi-agency meetings as appropriate.

Ensure own caseload is plannedand prioritised according to service targets and patient need.

Ensure patient held records arecompleted for each visit, and that EMIS inputting is completed at point ofcare.

Contribute to therequisitioning of supplies and equipment through electronic ordering systems.

Will be exposed to bodilyfluids, infected material, blood products on a daily basis, therefore mustutilise universal precautions and adhere to infection control policies.

Escalate any concerns to theTeam Leader for further clinical input.
