Band 3 Your Health Support Worker

**Main Duties & Responsibilities Working Pattern**: The service operates Monday to Sunday between 08:00 and 20:00 hours and you will be required to work shifts between these times. Key Areas of Responsibility To work within a multi-professional team in a community setting, providing, support, care, and treatment to individuals, within guidelines and protocols identified for the role. To provide support with registering unregistered patients to their nearest General Practice Surgery To promote Making Every Contact Count with every patient contact To promote the value of immunisations for both adults and children, as well as providing seasonal immunisations for adults including but not limited to COVID 19, Influenza To assess and sign post patients, or refer complex cases to Social Prescribers, to achieve quality of life and independence where possible To respond to infectious outbreaks as and when required, inclusive of swabbing and immunising To work with all health care professionals, and statutory/non statutory agencies to provide a seamless, integrated service to our service users To deliver high quality care in the community working with patients towards self-management and independence throughout the continuum of care. To participate in a team of colleagues supporting evidence based care.
To support the Band 6 with stock take of equipment and consumables on the vans. To undertake daily fridge monitoring and recording for the fridge on the van. To keep the patient at the centre of his or her care, ensuring patient and carer participation in decision making. To ensure the needs and wishes of the individual are documented.
To work to the National Protocol for any COVID 19 vaccinations. Undertake both routine and specially identified tasks for which the post holder has been trained and assessed as competent. Ensure proper disposal of clinical waste and change of PPE as per local and national guidelines. Ensure that if vaccination is delivered, dispose clinical and non-clinical waste in bins provided at the station and ensure surfaces are wiped down.
Wear adequate PPE equipment in line with current national guidelines and follow and adhere to infection control SOPs at all times. Be able to respond to and make judgements on complications or emergency situations, such as anaphylaxis and escalate to band 6. Respond to queries appropriately and liaise where necessary to gain further advice or information. A job plan will be created by your line manager in line with the service Blueprint and discussed with you.
It will be reviewed annually at your Performance and Development Review and updated as required. The main tasks you will spend your time doing are: - Clinical Direct, for instance phlebotomy; swabbing and immunising; supporting the Band 6 with community educational sessions; community outreach clinics. - Clinical Indirect, for instance patient documentation, travel to and from clinics, and liaison with other professionals. - Non-individual Patient Attributable, for instance attending MDT meetings, handover and safety huddles, caseload management, and equipment monitoring.
**The post holder will**: 1. Liaise with the Team Leader for clinics / rota and be flexible should the need change (e.g. during SURGE or dealing with an infectious disease outbreak) 2. Assist the Band 6 with planning the daily delivery of the clinics.
3. Undertake stock checks daily ensuring the van is adequately equipped for the clinics and ensuring the Band 6 is provided with a list of equipment / stock needs. 4. Undertake clinical elements of the Your Health services (blood samples / history taking / vaccinating) and record data in EMIS and feedback forms.
5. Where there are barriers to understanding, such as hearing impairment, mental capacity impairment and other difficulties in comprehension, explanations require adjustments in order to gain understanding, consent and concordance. 6. Seek patient consent and record this in the patient record.
7. Be aware of assistive technology and utilise where appropriate. 8. Participate in multi-disciplinary/multi-agency meetings as appropriate.
9. Ensure own caseload is planned and prioritised according to service targets and patient need. 10. Ensure patient held records are completed for each visit, and that EMIS/Pinnacle inputting is completed at point of care.
11. Will be exposed to bodily fluids, infected material, blood products on a daily basis, therefore must utilise universal precautions and adhere to infection control policies. 12. Escalate any concerns to Band 6 for further clinical input.
13. Follow all SWHC and National SOPs relevant to the services being offered under Your Health. 14. Responsible for the safe and effective use of equipment and other resources, ensuring equipment is maintained in good working order.
