and main responsibilities Work as a Health/Social Care Assistant to assist with the coordination and management of patients undergoing treatment at LNWHT to support patients both on and off Provide information to patients related to treatment/follow-up and signpost to appropriate healthcare providers within remit of role and Provide telephone advice and triage where appropriate, working within defined protocols and guidelines. Document and communicate outcomes as per service Support stratified Follow- Up for Cancer patients, be responsible for the management of IT systems that support this pathway ensuring that patients are entered and the pathway monitored. Triage telephone calls to the Cancer CNS telephone and liaise directly with patients, signpost and escalate to the CNS team as appropriate. Provide level 1 psychological support within own knowledge boundaries following appropriate Perform Holistic Needs Assessments using designated screening tools as To participate in collating patient surveys and develop action plans alongside the senior CNS. Support CNSs in developing and facilitating Health and Well Being programmes. This will involve contributing and delivering education to patient and carers, including information around health promotion, disease prevention and supporting self-management of their rehabilitation or long term Document all interactions both verbally and written with accuracy and in line with Trust policy and Demonstrate excellent communication skills both verbal and

Education/Qualifications
Essential criteria
GCSE/'O' level English and Mathematics NVQ Level 4 in Health or Social care, or equivalent experience/education Evidence of further study, courses related to healthcare or cancer care
Desirable criteria
Medical Terminology Course.
Knowledge & Experience
Essential criteria
Relevant Health or social care experience at Band 3 or equivalent, or transferable knowledge & experience of working with the public Understanding of key referral pathways to Allied Health Professionals to support plans of care Experience of using a spreadsheet and devising databases Experience in managing small projects Previous Involvement with Data collection systems and data analysis Experience of dealing with challenging situations
Desirable criteria
Previous experience of supporting patient/clients Previous experience in a cancer services setting or similar. Clinical audit knowledge,Previous experience of participation in audit Experience in service improvement Hospital PAS systems
