Job summary

Neighbourhood Working describes an integrated approach to managing patients, through a blended workforce that encompasses both health and social care; to include acute, voluntary and community sectors where barriers to working are negated, the health and wellbeing needs of the individual are at the centre of decision-making, care is proactive and not reactive, and services are provided in a timely manner.

This is a Community based post providing initial & on-going assessment and support to people referred through Primary Care and Neighbourhood Team practitioners. The post will sit within the Grantham Neighbourhood Team, supporting local populations in managing challenges experienced from physical and mental health conditions.

Main duties of the job

To provide a high standard of clinical care ensuring safe and effective assessment, treatment and comprehensive personalised care and support planning using highly developed clinical reasoning skills and appropriate assessment tools. This role will work with adults from 18 years upwards where a reduction in occupational function and performance is identified resulting from frailty syndromes. Some flexibility to work flexible hours will be required. To delegate aspects of the patient treatment plan to other members of the team, whilst maintaining overall responsibility for patients care, where appropriate. To ensure the clinical caseload and clinical practice of occupational therapists in the team is of the highest standard of clinical care. To manage referrals and service delivery in accordance with and in order to contribute towards the attainment of local organisational productivity requirements. To communicate and liaise with others to ensure appropriate assessments, interventions and reviews are in place. Work with individuals to assess their mental and physical health, recognise mental and physical illness and identify their related goals. Plan, implement, review and improve interventions to meet peoples identified needs and manage any associated risk. To assess carers and family's needs and signpost or refer them to the relevant support. To identify and act on safeguarding concerns using current pathways.

Job responsibilities

The post holder will be responsible for the comprehensive treatment & assessment of adults experiencing problems with occupational performance impacting on function resulting from frailty syndromes. Together with the individual they will assess needs, agree goals, implement intervention plans and evaluate progress. The role will support both the GP practice-based staff and members of the Neighbourhood Team to identify and support people to reduce the risk of unplanned hospital admissions and to effectively support those individuals in the community.

To independently support a defined frailty caseload with indirect supervision from the Primary Care Networks Neighbourhood Team Lead.

The post holder will provide specific occupational therapy interventions and professional advice and support to other members of the multi-disciplinary team.

The post holder will:

To be a pro-active member of the Integrated Neighbourhood Team and Southwest Primary Care Networks. To pro-actively engage with people deemed to be at risk of hospital admission or health deterioration. To proactively engage with frequent fliers those attending A&E and utilising OOH services. To pro-actively engage with Home First Teams to reduce length of stay in acute hospital settings. To pro-actively engage and provide expert advice to people living in care home settings and the care home teams. To be a key resource for Primary Care network colleagues. Person Specification

Qualifications

Essential

Degree or equivalent diploma in Occupational Therapy. Current Health and Care Professions Council (HCPC) Registration. Current full UK driving licence. Good standard of IT skills.

Desirable

Member of Royal College of Occupational Therapy. Member of special interest group. Clinical Educators qualification

Experience

Essential

Post graduate experience as an Occupational Therapist in rehabilitation with older people and or long term conditions and or mental health settings. Experience of working in the NHS or Social Care Environments. Experience in supervision, being supervised and supervising others.

Desirable

Experience of working in community settings. Experience of working in physical and mental health settings.

Specific Requirements

Essential

Ability to work across different sites and travel between locations to carry out work and to attend training and meetings within the Southwest locality and occasionally across the East Midlands area. Ability to respond and adapt to change. Ability to work under pressure. Excellent team worker. Ability to cover the needs of the role service including flexible working patterns.

Ability to critically appraise and evaluate clinical research, reports or papers. Specialist interest in service development area relating to older people and/or long-term conditions.
