We have an exciting opening for 2 Band 4 Assistant Practitioners to join our Oncology and Haematology Team working across our day case units. This is a rare opportunity to develop skills and competencies around the delivery of chemotherapy and other supportive treatments and to support the patients journey through clinical appointments.

We are looking for an established Band 4 Practitioner who has experience working within Cancer Services or who may be interested in a move to a new speciality.

The successful candidate will need to enjoy working with people and be able to maintain a professional approach in this sensitive environment. This post involves working alongside the Oncology and Haematology teams in their day case units through patient assessments, cannulation/venepuncture and the delivery of some medications. The Assistant Practitioner will co-ordinate and triage Chemotherapy patients prior to treatment

Main duties of the job

To provide safe, personal and effective care and support to patients, families and service users. To support the registered nurse, contributing to ongoing assessment and care planning for patients. To monitor the condition and undertake Holistic needs assessments of patients on a continual basis in partnership with colleagues, families and carers, referring to others for reassessment, when required

Detailed job description and main responsibilities

COMMUNICATION:

CARE DELIVERY:

  • Deliver holistic, person-centred nursing care
  • Actively engage with individuals, their families and/or carers undertaking Holistic needs assessments and contribute to care planning, by establishing their needs, wishes, preferences and choices and incorporating these into care planning
  • Ensure the rights of individuals are upheld and facilitate the resolution of any conflict arising
  • Act independently and in partnership with others to: ensure that the rights of individuals are not overlooked or compromised; and resolve conflict in situations where there may be refusal of care by individuals or their families
  • Deliver planned interventions/ care under the direction of a Registered Nurse, without direct supervision delivering care independently in line with an agreed and plan of care
  • Support healthcare professionals to assess, plan, deliver and evaluate care
  • Safely administer medication in accordance with local and national guidance
  • Undertake triaging of SACT patients the day before treatment checking bloods, clinic annotations using the triage tool on iQemo and liaising with the consultant if any discrepancies to provide a plan.
  • Giving the go ahead for treatments if bloods within normal parameters.
  • Provide care of PICC lines and removal when required.
  • Safeguard and protect vulnerable adults and children
  • Demonstrate the ability to treat all individuals, carers and colleagues with dignity and respect for their diversity, beliefs, culture, needs, values, privacy and preferences
