Job summary

We are looking for enthusiastic pharmacists with demonstrable competencies in hospital pharmacy practice, excellent communication skills and a demonstrable commitment to pharmaceutical care to join our team.

The Long Term Conditions rotations allow individual pharmacists to gain skills necessary to develop as a highly specialist clinical pharmacist specifically acute hospital and community settings, by working in a structured environment with the support of senior clinical pharmacists.

Main duties of the job

This post, in the Long Term Conditions rotational programme is aimed at pharmacists who have completed their Diploma and are looking to develop their clinical skills is specialist areas. This programme offers the following rotations: Gastroenterology, HIV, Anticoagulation, Immunotherapy (Dermatology & Allergy).

About us

As well as delivering ward-based clinical services, the candidates will have the opportunity to undertake Trust-based management courses, team management and organisational responsibilities ( staff planning, rostering and business planning), taking on a leadership role ( project supervision, training, mentoring and appraisal of junior staff members) and generating reports and leading investigations in line with Directorate priorities( root cause analysis and finance reporting)

Clinical Practice

To provide safe, clinically effective and cost efficient use of drugs in the patients as allocated.

To demonstrate and apply general pharmaceutical knowledge in core areas in order to plan, manage, monitor and review therapeutic programmes for patients allocated. To demonstrate and apply specialist clinical pharmacy knowledge in core areas in order to plan, manage, monitor and review therapeutic programmes for patients allocated. To manage patients pharmaceutical needs appropriately and make appropriate referrals. To follow legal, ethical, professional and employers codes of conduct. To ensure the quality of patient care. To demonstrate appropriate clinical reasoning and judgement. To ensure the clear and effective communication of patients pharmaceutical needs to other pharmacists and health care professionals both within the trust and in other health care environments. To demonstrate professional accountability to patients. To demonstrate awareness and commitment to the Trusts Clinical Governance Agenda.

Leadership

To act as a role model for junior pharmacists, pre-registration pharmacists, pharmacy technicians and students in the rotation.

To promote best practice. To implement agreed clinical governance initiatives. To develop clinical pharmacy services in line with local and national objectives. To work collaboratively with pharmacy and medical staff, the multidisciplinary team and management and to participate in the natural agenda for pharmacy services in accordance with the agreed objectives.

Management of service

To provide a clinical pharmacy service in accordance with the priorities set by the clinical team leader for each rotation in order to ensure safe, clinically effective and cost efficient use of drugs in these patients.

To ensure national and local agendas are delivered. To follow and conform to relevant standards of care. To identify and manage risks in relation to medicines use according to policy/protocol. To delegate duties / responsibilities appropriately to junior staff. To evaluate junior staff performance against identified criteria using departmental/Trust PDR tools and General Level Competence Framework. To promote Improvements in Working Lives. To provide evaluated pharmaceutical and financial information in accordance with the needs of the directorate. To identify changes in prescribing practices that may impact on the drugs budget. To demonstrate professional accountability. To be aware of and adhere to SFIs and the Trusts Corporate Governance commitments.

Evaluation of Service

To monitor clinical pharmacy in order to ensure safe, clinically effective and cost efficient use of drugs in these patients.

To document the workload of pharmacy services in line with departmental and corporate objectives. To document the quality of pharmacy services in line with departmental and corporate objectives. To undertake clinical audit as part of the multi-disciplinary audit process. To undertake assessments / evaluations as required and be able to apply the key principles of giving feedback.

Research and Service Development

To demonstrate a critical approach towards pharmacy practice in each rotation.

To integrate research evidence into practice. To develop clinical pathways. To actively seek to improve the ward and clinical services provided. To undertake practice research in accordance with the pharmacy research agenda. To assist with the supervision of undergraduate and postgraduate projects. Person Specification

Knowledge/Qualifications

Essential

Vocational masters degree (MPharm) Successful completion of short courses to the equivalent of a Postgraduate Diploma in General Pharmacy Practice, or equivalent experience. These would normally be equivalent to 180 Credit Accumulation and Transfer System (CATS) points Professional Registration with GPhC
