Job summary

TheSandwell Community Dementia Support Service (SCDSS) is commissioned by SandwellMetropolitan Council and the Black Country and West Birmingham ClinicalCommissioning Group. As lead provider, we are working alongside seven otherlocal organisations; Agewell CIC, Dementia Pathfinders CIC, West BromwichAfrican Caribbean Resource Centre, St Albans Community Association, BUDS, AgeUK Sandwell and Alzheimers Society to provide this service across SandwellMetropolitan Borough.

Main duties of the job

Supporting people with dementia and their carers inidentifying their needs, maintaining their independence, empowering them to make decisions and enabling them to remain in control of their lives. Triaging referrals, carrying out initialconsultations and in partnership with primary care, utilise appropriatescreening tools and co-develop care plans.

Job responsibilities

Working across Sandwell (Tipton,Rowley Regis, Oldbury, Wednesbury, West Bromwich and Smethwick) andneighbouring boroughs including Birmingham, Dudley, Wolverhampton and Walsall,you will ensure that you support people affected by dementia or people whomight be experiencing memory loss with the right and timely support that theyneed. You will engage with all clients using person centred principles andensuring that all individuals are supported appropriately using your skills andexpertise.

Person Specification

Qualifications

Essential

Dementia qualification Level 2 or 3 or equivalent. It is essential that the post holder acquires Level 3 whilst in post.

Experience

Essential

Demonstrable experience of working proactively in a busy environment and proactively supporting others in the team. Experience of CRM systems and database management. An understanding of client confidentiality (GDPR). Adult Safeguarding Procedures Lone Working Procedures Knowledge of the Mental Capacity Act

Desirable

Experience of working with voluntary and statutory bodies.
