Main duties of the job

Support and line manage the Community Offerteam, including Navigators and Administrators, providing support andsupervision on a regular basis. Working alongside other team members flexibly to meet the needs of the service, including some evening and weekend working. Supporting the team to work towards meeting servicetargets, by working closely with GPs and Link Workers, other providers, teammembers, volunteers, and community members.

Job responsibilities

Main duties and responsibilities:

To ensure that Navigators mobilise timely andeffective support to meet the needs of the clients, enabling clients tomaintain their independence, make informed decisions, and to help to preventunplanned hospital admissions.

To ensure that all referrals are offered basicinformation on benefits and to ensure that a referral to Welfare Rights isoffered and actioned in a timely way.

To assist with implementing scheme documentationand access criteria and an assessment tool to identify level of independence,monitoring tools for reporting.

To ensure that Navigators work with members of thepublic who are referred to the service, visiting them at home or preferredsetting as appropriate and maintaining a high standard of service to theclient.

To ensure that Carers are well supported across theservice areas by ensuring the team have access to up to date information onsupport available to Carers and how to access support.

To deal with day-to-day general enquiries ensuringpeople receive a prompt, professional and reliable service.

To take referrals from different sources, includingthe Single Access Point, and be responsible for providing/collectingappropriate information and/or actioning support from volunteers or providers signposting when and where appropriate, ensuring that the referral is actionedwithin the 72-hour timeframe as set by the funder.

To ensure all scheme activities are accuratelyrecorded to contribute to and demonstrate measurable outcomes, producingmonthly and quarterly reports as required by Service Manager, EMT and externalfunders.

Be prepared to travel to meetings, visiting clientsand attending meetings as required across Sandwell.

To organise and attend marketing events forprofessionals and members of the public as and when required.

To attend multi-disciplinary team meetings and toexplore other opportunities to ensure that information on how the service cansupport clients is communicated, and to adopt a problem-solving approach.

Post holder will be expected to undertake trainingor study as identified for the post, including undertaking Adult Safeguardingtraining and becoming designated DSL for Adults, working alongside other DSLtrained staff.

Experience of working with other partner organisations Experience of working with computers in Windows environment word processing, database. Experience of dealing with members of the public face to face.

Qualifications

Essential

Good standard general education and qualification (minimum level of GCSE) Competent in the use of IT software including databases Microsoft Word and Excel.

Appropriate qualifications within health/social sector - equivalent to level 3 and above
