An exciting opportunity has arisen for a unique band 4 Registered Nursing Associate working in Homeless Health Outreach in Blackburn with Darwen. We welcome applicants who are passionate and committed to working with individuals who are at risk of sleeping rough or experiencing homelessness. This is a great opportunity to work in a multi-agency partnership and develop close working relationship with a supportive Band 6 Senior Staff Nurse.
The successful candidate will work from satellite spaces and connect within community venues ( GP surgeries, hostels, pharmacies) where existing and potential service users live or are based..
This candidate will work closely with a multi-disciplinary team consisting of a Housing Outreach Team, Vulnerable Person's Case Co-ordinator (Calico), Engagement Worker and Lived Experience Worker (ELHT Alcohol Care Team). You will engage with Service Users and support them to access the right care to meet their presenting needs to ensure the best possible outcomes for the individual.
This outreach role will adopt making every contact count and they will support identify at risk individuals. This multi-agency approach will enable collaborative working across health, social care and voluntary sectors and helping individuals' problem solve through life challenges they are experiencing.
Main duties of the job
The successful candidate will provide safe, personal and effective care and support to patients, families and service users working within a multidisciplinary team and carrying out a diverse range of duties. They will support the registered nurses, contribute to ongoing assessment and care planning for patients. They will monitor the condition and health needs of patients on a continual basis in partnership with the multi-professional team. They will receive regular support and supervision from a Registered Nurse and be encouraged to take personal responsibility for clinical supervision and continued professional development plan. They will actively promote teamwork, within the clinical team in accordance with the Trust values and Code of Conduct. They will exercise professional accountability and responsibility using skills, knowledge and expertise in changing environments, across clinical boundaries and in unfamiliar situations as outlined within the NMC Code of Professional Conduct. They will work flexibly by working a rotational shift pattern including weekend and night duty to meet the needs of the ward and the wider network. They may be required to move clinical areas at either short notice or no notice in order to meet the changing needs of the service. They will participate in the process of internal rotation as and when required.
