Community Staff Nurse

Please note this post is based in North Lincolnshire .
The community nurse will work as part of an integrated 0-19 team, with support for young people up to the age 25 with special education needs and disabilities, contributing to family health needs assessment.
Working with families, children and young people age 5-19 years the post holder will work in partnership with the family and other agencies to develop implement and evaluate packages of care on an individual basis and with groups.
The community nurse will contribute to the wider public health agenda for Children and Young People through individual, group and community programmes of care.
Applications are welcomed from Registered Nurses and newly qualified nurses who are proactive and enthusiastic with excellent communication, interpersonal and organisational skills
Main duties of the job
The community staff nurse will work as part of an integrated team contributing to the family health needs assessment when working with families, children and young people age 0-19 years and up to 25 for young people with SEND. The post holder will work in partnership with the family and other agencies to develop, implement and evaluate packages of care on an individual basis and with groups.
