Community Health Nurse

Job Summary
The Community Staff Nurse will work as part of an integrated 0-19 team, contributing to family health needs assessment and providing care to families, children, and young people.
Working with families, children, and young people, the post holder will work in partnership with the family and other agencies to develop, implement, and evaluate packages of care on an individual basis and with groups.
The Community Nurse will contribute to the wider public health agenda for Children and Young People through individual, group, and community programmes of care.
Applications are welcomed from Registered Nurses and newly qualified nurses who are proactive and enthusiastic with excellent communication, interpersonal, and organisational skills.
Main Duties of the Job
The Community Staff Nurse will work as part of an integrated team, contributing to the family health needs assessment when working with families, children, and young people age 0-19 years and up to 25 for young people with SEND.
The post holder will work in partnership with the family and other agencies to develop, implement, and evaluate packages of care on an individual basis and with groups.
