We are excited to be able to advertise for a Service Lead for our Community Nursing and Therapy planned care services in South Norfolk, joining our South management team. This rewarding post is suitable for an experienced, highly motivated individual, who is open to exploring and supporting service development.

We provide services across the 4 South Norfolk PCN areas and have community teams and bases across our patch whom you would closely with on a daily basis. We are privileged to work with people in their own homes and communities and work alongside other community partners and agencies.

Main duties of the job

Responsibility for the effective management and delivery of community and clinic based services within the Place. This includes addressing any complex performance issues, financial issues and clinical risks, highlighting exceptions and escalating appropriately to the Head of PCN Health

The role requires a high level of managerial knowledge to underpin the need to analyse and respond appropriately to complex organisational and staffing issues, providing leadership to the Clinical Operational Managers / Clinical Leads.

The post is responsible for the effective flow of patients between primary, secondary and intermediate care, working closely with GPs and adult social care.

To ensure that services are delivered efficiently and effectively to a high standard that will meet commissioning and CQC standards.

Promote good working relationships with both internal and external system partners.

Job responsibilities

Organisational- Lead on and ensure the implementation of service improvement initiatives in line with national guidelines and organisational objectives. To include the use of advanced communication skills to deliver highly contentious and sensitive information to staff at all levels in order to promote engagement and cooperation.- Be responsible for ensuring sufficient numbers of staff to deliver required service levels on a daily and monthly basis, using advanced planning and analytical skills. To include deciding on the most appropriate action in response to inadequate staffing, taking into consideration constraints such as finance, skill mix, complexity of patient and risk.- Management of integrated teams / services within the Place.- To be responsible for monitoring and analysing service performance activity data across all locality services to ensure that agreed internal and external Key Performance Indicators and deadlines are met. To include promoting and ensuring compliance from Clinical Operational Managers / Clinical Leads.- To communicate service-related information to senior trust managers both face-to-face and in the written form.- To use advanced communication skills to manage the expectation of service users, balancing the appropriate use of staffing and financial resources and adherence to the agreed service specification, with the need to maintaining good relationships.- Support and develop research opportunities within the Place, using the results for effective service improvement.- To create, maintain and enhance wide ranging and effective systems of communication and working relationships within and outside NCH&C and with service users.- Overall responsible for ensuring timely recruitment, ensuring human resource policies are adhered to.- To participate as a senior manager on hearing panels across all localities and instigate and conduct employee investigations as required.- Manage incidents in conjunction with Operational and Quality team.- Contribute to service planning by ensuring workforce issues are identified and addressed in a timely manner. To include making recommendations to the Head of PCN Health and Operations Director regarding skill mix and recruitment needs based on the consideration of a variety of information including performance, waiting lists, complaints, risks raised viaDatix, service user and staff feedback.- Ensure that service users receive care closer to home, in a place that is conductive to their wellbeing.- Identify and implement service development taking into consideration commissioning intentions, national directives and clinical guidelines ( NICE). This will include the development of multiagency pathways of care.- Engage staff in audit, research and evaluation.- To monitor allocated budgets ensuring appropriate use and balance of funds.- Represent the Place and Norfolk Community Health and Care at a senior level at local, regional and national events and at multiagency meetings.- To work collaboratively with the operational and quality team in the development of a comprehensive risk register for the Place. To include taking and delegating actions to mitigate risk and escalating appropriately to the Head of PCN Health and Operations Director, sharing top risks at business unit and trust-wide meetings.- To explore all opportunities for integration including joint roles where appropriate and to propose options to the Head of PCN Health.- To work collaboratively with the operational and quality teams regarding both formal and informal complaints raised in relation to local services, adhering to the trust complaints policy and to ensure learning is shared appropriately across the locality.- To identify opportunities for new business, funding and income and propose to the Head of PCN Health and Operations Director. Develop Business Cases working in conjunction with the business development Team.- To work on projects at a strategic and operational level as required by the Trust and Place. To include implementation and evaluation of effectiveness via audits and the presentation of results.

Clinical and Professional leadership- Promote a learning culture by promoting and enabling professional supervision, ensuring all staff have individual performance reviews and personal development plans.- Support transformational leadership for staff; identifying, developing and empowering leaders at all levels.- Identify and lead clinical and service development opportunities, creating an environment that fosters positive change and promotes the implementation of access to health services.- Set an example in working to corporate standards and values and ensuring staff are aware of and adhere to the behavioural framework.- Provide advice internally and externally act as coach / mentor for relevant members of staff.

Working Relationships- To develop strong relationships with all levels of the NHS and key local system partners including ICB, General Practitioners, Norfolk Adult Social Care, District Councils, Voluntary Organisations, internal and external auditors, NHS Trusts and other providers including the voluntary sector.- Establish working relationships with GPs, CCGs and other commissioners of services.- To ensure and maintain involvement of clinicians in decision-making regarding service planning and delivery.- To ensure effective liaison with acute hospitals to ensure timely transfer of care along the relevant care pathway.

Qualifications

Essential

Registered Adult Nurse Educated to Masters level or equivalent experience working at a senior level Evidence of Continuing Professional Development Management or Leadership course

Essential

Extensive experience of leading and managing a team at band 7 level. Good understanding of the background and aims of current health and social care policy and appreciate the implications of this on community service delivery. Experience of analysing and interpreting data to produce explanatory reports, identify trends to deliver service development and improvement. Report writing to ensure accuracy of information and translation. Able to provide high standards of clinical care.

Desirable

Experience of working at band 7 level within a community setting. Experience of effectively leading a multidisciplinary/ multiagency team at band 7 level.
