Job overview

Hi, I’m Hannah and I have been working as a Band Specialist Respiratory Nurse in the Community for years now and honestly, I can’t imagine working anywhere else. My main role is to support patients in their own homes and prevent their chronic respiratory condition from having them admitted to hospital. This may involve, full clinical assessments, oxygen reviews, medications management, end of life discussions and much more.

 We work closely with patients to support this, not only when they are acutely exacerbating but also when they are stable. I work closely with GP’s, consultants, physiotherapists, support workers and other nursing teams to ensure our patients receive high standards of holistic care.

I was initially nervous about leaving the safety of secondary care but soon realised that although we work autonomously in the community there is always support in the form of colleagues and peers at the end of the phone if needed. I enjoy the autonomy this role allows and am able to use a wide range of nursing skills, with no two days being the same. 

We are looking for a qualified and registered nurse with respiratory background, but our goal is to find someone with passion and interest in respiratory conditions and supporting people in their own homes. Therefore, we are happy to consider candidates that require further community and/or respiratory training.

Main duties of the job

The post entails providing expert supportive care for respiratory patients, by helping to prevent avoidable hospital attendances, providing facilitated support following hospital admission and the promotion of focused self management, which will include reviewing those with home oxygen therapy.

The successful candidate will also support the team in the delivery of our Pulmonary Rehabilitation Programme. 

The successful candidate must be capable of working autonomously and work with the support of CPFT respiratory colleagues, in providing a seamless service of care. The post holder will work in collaboration with other services across the CPFT and other stakeholders in order to make a positive impact on the health and well-being of patients with chronic respiratory disease. They will also act as a community respiratory care resource for patients and other health professionals.

It is anticipated that the successful candidate will have respiratory and/or community experience, however, training and support is available for candidates with the potential to develop.
