Job summary

This is an exciting opportunity to work within a high functioning, well respected team. The Rapid Access Service (RAS) is an ever evolving, fast paced, community service, comprising of three pathways; 'Urgent Community Response', 'Discharge to Assess', and 'Virtual Ward'. You will be working closely with members of an MDT to reduce hospital admissions, and support timely discharges from hospitals.

The caseload is highly varied and includes delirium, TLSO and C-Spine braces, frailty and falls. You will support junior staff in the management of these complex patient's, whilst also developing partnerships with GPs, LAS and acute hospitals to provide an urgent response and risk assessment for people in their own homes. Within this setting there is a key focus on being able to identify a deteriorating patient, and escalating appropriately when required. There may be many barriers to treatment in the community, requiring complex problem solving and working with other external agencies to enable to the best outcome for these people.

You will have the opportunity to work within the therapy 'Falls Urgent Response' pathway for non injured fallers in the community. This will comprise of using specialist equipment to assist patients off the floor, and completing thorough assessments to ensure the patient can remain at home.

This is a 7 day service consisting of varying shift patterns between 9am-10pm.Willing to discuss flexible working, and happy to arrange an informal visit.

Main duties of the job

As a member of the Rapid Access Service, you will be expected to be a skilled Occupational Therapist who is an excellent communicator, is flexible and able to adapt to the needs of both patient's and service. It is essential you are skilled in complex holistic assessments (functional, environmental, cognitive) to ensure patients are safe to remain at home.

This is a challenging and highly rewarding role and will enable you to develop your skills in the holistic treatment within a variety of areas. These include knowledge of medications and their impact on function; being competent in taking observations and completing NEWS2 scores on patient's, interpreting these scores and identifying and escalating when a patient is unwell; being exposed to an array of medical and long-term conditions in people over the age of 18; identifying the need for a GP or specialist review as part of diagnostics, and being competent in liaising directly with the appropriate specialist community teams to ensure seamless care.

Due to the unpredictable nature of the service it, flexibility and and quick decision making is key for success. Assessing and issuing emergency equipment and setting up emergency care within complex situations is crucial. As part of the holistic assessment and safe set-up, you will identify any further longer term needs, and make the relevant referrals to those services community neuro, rehab, or voluntary services.

The service is purely community based and has the privilege of being invited into the homes of Camden residents from as far north as the borders of Golders Green and as far south as Holborn and everywhere in between.

Job responsibilities

Due to a 7-day working pattern, you will have a shared team caseload. You will provide complex assessments and interventions and will make high impact clinical decisions. You will be expected to work autonomously whilst being very well supported by a dynamic and approachable team, who will encourage growth and leadership in your role. As part of clinical development, you will have the opportunity to be upskilled in venepuncture to support clinical decision making around patients treatment and care. You will also have the opportunity to develop skills in supervising junior staff and students, and be encouraged to identify and participate in service development and QI projects.

As part of your role you will also have the opportunity to be Clinician of The Day (COD). This consists of screening all referrals for all pathways. This is a key leadership role in which the COD is required to challenge where necessary the appropriateness of referrals, and ensure all the relevant information is gained to ensure safety for all parties. The COD co-ordinates and oversees the therapists caseload on that given day, and allocates new referrals as they are discharged from hospital. This is a balancing skill in order to ensure the team have capacity and the discharge is well co-ordinated in line with the integrated discharge is much scope for development of clinical skills within this role, and our team has a proud history of a high staff retention rate and have experience of developing and promoting therapists within the team.

You will have timely management and clinical supervision and annual appraisals with additional training opportunities in line with this.

We will aim to complete shortlisting 3 days from the close date, and interviews aim to take place for 7-10 days thereafter.

Degree / Diploma in Occupational Therapy Registration with HCPC as an Occupational Therapist Worked in the NHS or equivalent healthcare service
