Job summary
An exciting opportunity has arisen to join the diabetes team at Liverpool Heart and Chest Hospital. This is a part time band 6 role which will include inpatient diabetes management and diabetes outpatients clinics. It's an ideal development opportunity to extend your skills and gain experience in diabetes care. You should be enthusiastic and self motivated, with a demonstrated interest in diabetes care.
You must possess excellent communication and interpersonal skills and can demonstrate good leadership qualities to ensure our patients receive effective, efficient, compassionate and safe care.
Main duties of the job
To contribute to the work of the diabetes nursing team in providing clinical support and advice to patients, families, carers and other healthcare professionals
To provide education and support to patients and families/carers in an attempt to create an understanding of the treatment and management of diabetes and its complications.
To be an active and collaborative member of the diabetes team and the wider multi-disciplinary team involved in the diabetes services.
To take an active role in the providing education and training to staff across the Trust.

Job description
Job responsibilities
To undertake timely clinical assessments of patients referred to the service using an expert level of clinical knowledge and provide advice on the most appropriate management. This will include advice on most suitable insulin delivery systems and oral therapy.
To undertake patient assessments and work with practitioners to identify and plan individualised care for patients with diabetes.
Be an integral member of the multidisciplinary team in advising on the admission of pre-operative patients with diabetes.
Provide advice, guidance and support to patients who are newly diagnosed with Diabetes.
On discharge ensure understanding of medications and treatment plans by utilising teach back patient education process to assist with self-management of Diabetes.
To act as a resource for other professional groups, teams and individuals providing care for people with Diabetes.
Organise and supervise opportunities for nursing staff to enhance and develop their knowledge and skills in the field of Diabetes.
To develop, in conjunction with the multidisciplinary team, protocols, documentation systems, standards and guidelines to enable best practice for the care of patients with Diabetes.
To represent the Diabetes Team at internal and external meetings as required and to deputise for the Diabetes Service Lead when needed.

Essential
Registered nurse with current NMC registration Mentorship/Teaching qualification or experience Postgraduate qualification or certificate in Diabetes or a willingness to undertake Evidence of continuing professional development
Desirable
Non -Medical Prescribing Qualification (working towards or a willingness to undertake)
Knowledge & Expererience
Essential
3 years post registration experience Substantial post registration experience working with patients with Diabetes Mellitus Evidence of autonomous practice Experience of providing patients centred education and support Up to date knowledge of current issues surrounding Diabetes including current guidance and best practice Knowledge of current treatments of Diabetes
Desirable
Previous experience of providing training to staff members Experience of audits Evidence of clinical leadership / change management Experience of providing clinical supervision to nursing students and junior members of staff.
Skills & Abilities
