Job summary
There is an exciting opportunity for a digital & transformation lead to join us at Hillton Primary Care Network, on a part time basis for hours per week, to lead us as we transform the way that we support our patients, the way our staff are able to work & the sustainability of our GP services, through the adoption or optimisation of new technology & other initiatives. Travel will be required between all 3 surgeries within the PCN.
If you are an experienced project manager or transformation lead, looking for an exciting new challenge, then we would love to hear from you. The ideal candidate will have experience of working in an NHS environment.
Main duties of the job
Overseeing the adoption/optimisation of new technology to enhance patient access & experience, to increase PCN productivity Building relationships & facilitating collaboration between practices & the wider system, to support the delivery of care to patients (including shared appointments between practices to aid delivery of enhanced access) Leading an improvement approach to change, including building capability for quality improvement within the PCN, & system-wide approaches to problem solving; Reviewing and improving the PCNs digital maturity Using data, & improving data quality, to: understand demand, capacity and activity, and to drive improvements in: patient experience of access operational efficiency including better matching capacity to need staff experience at work support population health management support understanding of the type and intensity of support & training needs of the PCN, coordinate this support, including through OD programmes facilitate clinically-led innovation & the effective adoption of improvement initiatives, including integrated working at neighbourhood/ place level to improve access to services for patients.
