Job summary

We have an exciting opportunity for several Advanced Clinical Practitioners to join our Out of Hours team at our Barrow-In-Furness and Kendal sites. This is on a Salaried basis.

The role is critical in providing high-quality care out of hours, ensuring our patients receive timely and effective care.

Up to 20 hours per week are available for each site and these are primarily night shifts.

Main duties of the job

The post holderwill work as a member of the clinical team. The post holder will be responsiblefor recognising, negotiating and implementing processes to enhance the deliveryof care in the Treatment centres.

Patient centredcare will be delivered by assessing patients with undifferentiated andundiagnosed problems; initiating treatment, investigations, advice andmanagement of problems using professional autonomous decision making processes,liaising when appropriate with other healthcare professionals and serviceproviders to ensure safe and effective patient care.

The post holderwill contribute to development and delivery of a high quality innovativeservice to the community through the implementation of evidence based clinicalpractice, supported by continual professional development andmentorship/supervision.

Communication

1. Communicates effectively as a member ofa multi-disciplinary team.

a. Refers when necessary to generalpractitioners, Paramedics, Accident and Emergency, Secondary Care, Districtnurses, Night rapid response nurses, Emergency social workers, Emergency mentalhealth team, and Emergency dental services.

2. With Primary care nurses giving support,advice and leadership.

Clinical Skills

1. Works independently as an AdvancedClinical Practitioner within the scope of professional practice.

a. Functions at a high level to a carry out clinicaldecision making such as diagnosing and discharging patients with minimal inputfrom a medical practitioner remaining with the scope of professional practice.

b. Practices as an autonomous practitionerwhere possible with non-medical prescribing authority.

2. Functions at a high level in relation tocommunication skills through patient consultation and nursing assessment ofphysical, psychological and social needs.

a. Takes an accurate history, buildingrapport towards a therapeutic relationship.

b. Assess, plan and implements patientcentred plan of care for patients with in the out of hours treatment centres,independently discharge patients from your care and refer when necessary toother healthcare professionals for further specialised care.

3. Demonstrates clinical competence in theassessment and management of patients of all ages from infants to the elderlyand management of lifelong illness such as diabetes, asthma and chronicobstructive pulmonary disease during the time of acute illness and managingminor illness.

4. Adapts to change within workingsituation managing unscheduled emergency situations.

a. Basic life support, severe breathingproblems, serious cardiovascular problems, collapsed patients and seriously illpatients.

5. Performs investigations to reach adifferential diagnosis using core clinical skill and competence. Initiatetreatment including diagnostic investigations.

a. Recognition of abnormal vital signsobserving paediatric and adult respiratory rate and effort, heart rate rhythmand volume, temperature and blood pressures as appropriate.

b. Performs investigatory proceduresincluding; Performing ECGs, Urinalysis, Blood Glucose monitoring,Venepuncture, measuring and interpreting Peak Flow.

6. Demonstrates competence in examinationtechniques

a. Examination of the Ear Nose and Throatsystem

b. Basic Neurological examination ofHeadaches and Head injury

c. Basic Muscular-skeletal examination inthe assessment of injury

d. Basic eye examination in the assessmentof injury, foreign body and infection.

7. Performs safe manual handlingtechniques.
