Job Title : Community Psychiatric Nurse - Band 6

Trust : Devon Partnership NHS Trust

Location: Trust also covers areas in Exeter, Torquay, Ivybridge, Barnstaple. Main hospitals are exeter and Torquay.

Pay Rates: £ to £

Shift Patterns: 7:30am to 3:30pm, 7:30am to 12:30pm, 9am to 5pm, 12:30pm to 8:30pm, 3:30pm to 8:30pm, 8pm to 8am,

What you can add:

Providing robust assessment of patients (by telephone or face-to-face) who have been referred due to presenting with mental health difficulties. Identifying appropriate care pathways and ensuring the least restrictive option to meet the needs of the patient and ensuring their wellbeing. Signposting to appropriate care providers (including primary and tertiary care). Raising concerns to senior staff for support and management. Completingdocumentation and planning care for patients. Presenting cases to Multi-Disciplinary Team for discussion around care pathway. Liaison with colleagues nursing, occupational therapy, medics, psychology and admin.

The role will involve, but not be limited to you performing the above.

What you'll need:

A minimum of 6 months paid experienceas a Community Psychiatric Nursein anUKhospital environment within the last 3years Comply with NMC Professional Registration and keep up to date with current nursingpractices *Full UK driving licence and own vehicle.

*Depending on the Trust, you may require variable additional certification and skills.
