About the Role:
We are seeking a highly skilled and dedicated Community Nurse to join our team in Exeter. As a Community Nurse, you will provide holistic patient-centered care for patients who are housebound, with complex, chronic, and long-term conditions.
You will work closely with the wider community multidisciplinary team to promote admission avoidance and empower patients to receive their nursing care at home. Your key responsibilities will include assessing, providing, and reviewing nursing care, advice, and information to patients and carers within the home environment or appropriate care setting.
As a Community Nurse, you will be expected to maintain patients in their preferred place of care, enabling them to maximize their independence and optimize their quality of life. You will work under the direction of the Specialist Nurse (Community) and the Nurse Team Manager (Community), using your skills and knowledge as a competent Registered Nurse.
